In order to improve access to adult community care services, Falkirk Council has changed the way our teams work.
We hope these changes will mean we can better manage our resources to ensure that the public and partners continue to receive services quickly and more effectively.
We have developed a pilot Intake Team that, from 14 March 2011, will be the first point of contact for social work service users and carers with community care needs.
The Intake Team will cover the Grahamston, Camelon, Stenhousemuir and Denny areas and will be based at the Camelon Office.
This team will provide assessments and arrange any services required to meet service users assessed needs, in line with our eligibility criteria.
The Intake Team will work with service users and carers in the short-term and will transfer involvement over to the long-term teams if there is a need for continuing support.
If service users live in the Grahamston and Camelon areas ongoing support will be provided by the Central Long-term Team. If service users live in the Denny and Stenhousemuir areas ongoing support will be provided by the West Long-term Team.

This pilot will give Falkirk Council an opportunity to see if the Intake and Long-term teams are the best way forward to deliver adult community care services.
The pilot will be reviewed over a 6 month period from April 2011 and our findings will be presented to Housing and Social Care Committee and will be published on our website.
Your views on this are very important to us. We will speak to some people about their experiences in using these services during the pilot period.

If you feel that you need a community care service, please contact your local social work office and ask to speak to a member of the community care team.
From this initial enquiry, we will determine if you require a visit from a social worker, occupational therapist, occupational therapy assistant, social care officer or community care worker, and we establish the urgency of your needs.
Confidentiality
Social Work staff members recognise the need for confidentiality. They may find it helpful to discuss your needs with other care professionals or members of your family but they will ask for your permission first.
If there are any details you do not want to be disclosed, they will normally respect your wishes, unless someone would be placed at risk by doing so.
