Your local health and social care services are provided by Falkirk Health and Social Care Partnership (HSCP).

The Partnership is made up of staff working in Falkirk Council and NHS Forth Valley, along with partners in the Third and Independent sectors.

The Integration Joint Board (IJB) oversees the HSCP. The IJB is responsible for overseeing the planning, management and delivery of relevant health and social care services. The Chief Officer for the Partnership works closely with health and council staff to plan and direct all of the delegated health and social care services. The IJB is made up of representatives from Falkirk Council, NHS Forth Valley, staff representatives, service users, carers and Third Sector representatives.

Health and Social Integration is about putting people first and ensuring that we tailor services to meet your individual needs. This will make it easer and quicker for you to access the services and the support you require. We will focus on the outcomes you want to achieve. This will help you live safely and independently in your home, for as long as possible.

The Partnership will work to ensure that health and social care provisions across Falkirk is joined up and seamless, especially for people with longer term and often complex needs, many of whom are older.

Integration of health and social care is part of the Scottish Government's programme to improve health and wellbeing outcomes.
