Help for children with disabilities
If you have a child or young person who has a significant and substantial disability, we may be able to offer support.
Our Aims
Connect families with the right services
Work together to create equal opportunities
Deliver high quality, responsive support
Tailor care to each child's individual needs
Our multidisciplinary team provides three key services:
Social Workers can:
- Signpost to local support
- Complete a Section 23 (outcomes focused assessment) to help support your child and your family
- Following the completion of a section 23 outcomes could include:
- short breaks away from home for your child
- care packages at home to help with everyday tasks for your child
- Support families who choose to access Self Directed Support to manage their own care arrangements
The Activity Scheme Coordinator can:
- Offer access to our Activity Scheme during school holidays
- Offer access to youth clubs and art groups
- Provide social and recreational opportunities for your child
- Offer a Befriending Service throughout the year to give young people leisure and recreational experiences
To access Activity Scheme, Befriending, youth clubs and art groups, you will need to complete an application form and will be assessed against the criteria.
